After a time, the term Labor Coach fell from favor as politically incorrect, because to coach a woman in labor suggests that the particular woman can't give birth without help, without coaching. 
Today we approach full circle with a switch: Doula is now politically incorrect because the source is ancient Greek and means a female slave to another female.

Trauma during childhood induces permanent changes to gene regulation putting abused children at high-risk of mood and anxiety disorders. (1)

The stress hormone system may suffer long term changes in DNA methylationn resulting in dysregulation of the stress hormone system.

Genetic predisposition to psychiatric disorders greatly influence the interaction between stress, environment, trauma, and the development of psychiatric illnesses later in life.

Torsten Klengel, a scientist at the Max Planck Institute of Psychiatry in Munich, explains the findings of  research and study on childhood trauma and adult mental illness as follows: 

"Depending on genetic predisposition, childhood trauma can leave permanent epigenetic marks on the DNA, further de-repressing FKBP5 transcription. The consequence is a permanent dysregulation of the victim's stress hormone system, which can ultimately lead to psychiatric illness. Decisive for victims of childhood abuse, however, is that the stress-induced epigenetic changes can only occur if their DNA has a specific sequence."

Preterm birth is defined as birth before 37 weeks of completed gestation. 

Preterm birth is the leading cause of newborn death, and the second leading cause of infant death. (1)


Babies surviving premature birth classically face a lifetime of health challenges, including breathing problems, cerebral palsy, motor and intellectual disabilities and others. (2)

Prevention of preterm births of 58,000 babies per year is thought possible just by lowering preterm birth rates of 39 countries across the world by 2015.(3)

Five proven interventions to prevent premature birth are sited.

These five interventions include:

  • "eliminating early cesarean deliveries and inductions of labor unless medically necessary
  • decreasing multiple embryo transfers during assisted reproductive technologies;
  •  helping women quit smoking;
  •  providing progesterone supplementation to women with high risk pregnancies;
  • cervical cerclage for high-risk women with short cervix." (4)

Five alternative cost effective interventions to prevent premature birth and eliminate ethnic health disparities in outcome of pregnancy and maternal and newborn health:

The newborn pictured is an exception, and not by accident. He was born to a teenage mother, age 18, and high risk by all standards. Yet he was born weighing over 9 pounds and healthy. Today he is 8 years old and academically gifted.
He is an exception because across the United States of America, the general rate of preterm birth has decreased for Caucasian Americans, yet in the state of California, the rate of preterm births has increased for African Americans. 
African American babies are born too early and too sickly to live on their own without assistance of technology provided by neonatal intensive care units (nicu).  

Eat to meet the nutritional demands of pregnancy from the inside out
Nutrition supplements for all  pregnant mothers
Allow pregnancy to advance spontaneously to full term 
Provide comprehensive perinatal education based in nutrition for all pregnant mothers.
Avoid tobacco, alcohol, and street drugs.

A small percent of women show signs of postpartum post traumatic stress disorder (PTSD) after giving birth.

According to Science Daily, Aug 8, 2012, "Post-Traumatic Stress Disorder (PTSD) develops in individuals who experience highly traumatizing situations such as terrorist attacks and car accidents, but symptoms can also come about after normal life events -- including childbirth." (1)

Long taught by midwife, founder of the Association for Childbirth at Home, International, founder and activist, Tonya Brooks, overwhelming pain in childbirth may contribute  to the development of some degree of PTSD symptoms from partial to full blown PTSD.

"It has been suggested that the intense experience of pain can lead to an event perceived as traumatic." (2)

Factors contributing to childbirth related PTSD include
-fear of labor itself
-fear of pain
-fear of death
-fear of danger to self
-fear of danger to preborn baby
-natural childbirth without pain relief
-body image issues
-discomfort at being undressed during labor
-elective cesarean birth
-complication of present and earlier pregnancies.

Certain factors are known to be associated with development of postpartum PTSD.

These risk factors include events surrounding the birth, subjective perception of loss of control during labor; use of invasive procedures including manual and cesarean; sexual abuse; previous psychiatric treatment; fear of loosing the baby; quality of social support; and attachment style.(3)
